Job Description

Public Sector People (PSP) are currently looking to speak to Case Manager (Aged Care) to join a Local Government from April to July 2019 in the inner south east.

About the role:
  • 3 month contract - potential extension beyond this 
  • Inner south east 
  • Band 6 
General Duties Include:
  • Work in partnership with clients and carers to assess their needs, set goals and develop care plans which acknowledge their abilities, strengths and desires
  • Maintain up to date knowledge of community services/programs and refer to other services as appropriate
  • Monitor and review clients and services to ensure that goals are achieved and quality services are delivered
  • Respond to changes in client or carer needs, changing care plans as necessary whilst maintaining continuity of care
  • Well developed knowledge of health and community services for the aged and people with disabilities, including Home and Community Care and Commonwealth care packages
  • Demonstrated understanding of person-centered practice
  • Well developed ability to use Information Technology, including MS
Requirements include:
  • Tertiary qualifications in the health or social sciences
  • Post-graduate studies in case management or other relevant area highly regarded
  • Experience in delivering high quality case management services to the aged and people with disabilities, preferably in delivering care packages using a brokerage model
