About the role: 

Public Sector People are currently inviting applications for the role of Service Coordination Officer to join Council in Melbourne's south east working with successful AccessCare team. 
As the successful candidate you will be responsible for developing & maintaining positive working relationships with Carers, Family, Service Providers & AccessCare Southern Staff. Day to day you will manage daily enquiries to service changes whilst working in partnership with Case Managers to meet goals and care plans of the consumer and under guidance from Case Managers resolve customer complaints.

Key Requirements: 
  • Extensive experience working in service coordination role or similar 
  • Experience working in a community service environment
  • Experience using a system CareLink + or similar
  • Ability to build cooperative working relationships with other service providers
  • Victorian Police Check - or ability to obtain if successful
